These Terms of Service ("Terms") govern your use of our website, booking platform, employer portal, and compliance services — including mobile and in-location phlebotomy, drug and alcohol testing, fingerprinting, DNA testing, background checks, and related services. By using our services, you agree to these Terms in full.
1. Acceptance of Terms
By accessing or using the Apex Compliance Corp website, booking platform, employer portal, or any of our mobile or in-location services, you agree to be bound by these Terms of Service. If you do not agree with any part of these Terms, you must not use our services. Your continued use of our services constitutes acceptance of any updated Terms.
2. Services Provided
Apex Compliance Corp provides compliance and testing services, including but not limited to: • DOT and Non-DOT drug and alcohol testing (urine, oral fluid, hair follicle) • Ink fingerprinting (FD-258 cards) and electronic fingerprinting • Background checks and employment verification • DNA testing (paternity and relationship) • I-9 employment eligibility verification • Mobile phlebotomy and specimen collection services • Medical Review Officer (MRO) services coordination Services may be performed at our Tempe, AZ location or via mobile dispatch to your specified address. Availability of specific services may vary.
3. Appointments and Scheduling
When you book an appointment through our platform, you are requesting a service time. All appointments are subject to availability and confirmation by our team. We reserve the right to reschedule or cancel appointments due to circumstances beyond our control, including but not limited to weather, staffing, or facility issues. For mobile services, you must provide a safe, accessible, and appropriate environment for our technicians to perform services. If a location is deemed unsafe or inaccessible, our technician may reschedule the appointment, and a travel fee may apply.
4. Your Responsibilities
As a user of our services, you agree to: • Provide accurate, complete, and truthful information when booking appointments and creating accounts • Bring valid government-issued photo identification to all appointments • Arrive on time for scheduled appointments or provide at least 24 hours' notice for cancellations • For drug testing, follow all pre-test instructions provided (e.g., abstaining from certain substances, bringing prescription documentation) • Ensure the service location for mobile appointments is safe, private, and sanitary • Authorize the release of test results to designated recipients where applicable • Comply with all applicable laws and regulations For employer contract clients: you agree to maintain accurate employee rosters and authorize only designated personnel to book appointments on behalf of your company.
5. Cancellations and No-Shows
Appointments may be cancelled or rescheduled without charge with at least 24 hours' notice. Cancellations made less than 24 hours before the scheduled appointment, or failure to appear (no-shows), may be subject to a cancellation fee of up to $50.00. For mobile services, if our technician arrives at the scheduled location and is unable to perform services due to client unavailability, unsafe conditions, or incomplete information, a dispatch and travel fee will apply.
6. Fees, Billing, and Payment
Service fees are displayed at the time of booking or provided in your employer service agreement. Unless otherwise stated in a contract: • Individual appointments require payment at the time of booking or service • Employer contract clients are billed according to the terms of their service agreement • Invoices are due within 30 days of the invoice date unless otherwise agreed • Overdue invoices may be subject to late fees and suspension of services All fees are quoted in U.S. dollars. We reserve the right to adjust pricing with reasonable notice.
7. Test Results and Reporting
Test results are processed by accredited laboratories and reviewed by a qualified Medical Review Officer (MRO) where applicable. Results are released only to authorized recipients in accordance with applicable laws, regulations, and your signed authorizations. Apex Compliance Corp does not guarantee specific test outcomes. Turnaround times are estimates and may vary based on laboratory processing, MRO review, and other factors outside our control. We are not liable for delays in result delivery caused by third-party laboratories or MROs.

We handle all personal and health information in accordance with our Privacy Policy and applicable laws, including HIPAA. You acknowledge that test results and related information may be disclosed to your employer or other authorized parties as permitted by your authorization and applicable law.
